DECLARATION
I, the undersigned, hereby apply for / renew my membership with APPNA British Columbia Chapter and solemnly declare that:
Professional & Legal Standing
I hold valid and current professional credentials and licensure where applicable
I will provide proof of licensure upon request
I declare that:
I have no criminal convictions or ongoing legal proceedings
OR I have disclosed all relevant legal/disciplinary matters to the Membership Committee
- Ethical Conduct & Integrity
I agree to uphold the highest standards of professional ethics
I have not engaged in fraud, harassment, discrimination, or professional misconduct
I agree to abide by APPNA’s Code of Ethics and all applicable regulations
- Organizational Commitment
I will support the mission, objectives, and best interests of APPNA BC Chapter
I will not engage in actions that undermine the organization’s integrity or reputation
I will respect decisions made by the Board of Directors
- Good Standing Requirements
I understand that maintaining good standing requires:
Timely payment of dues
Maintaining valid licensure (if applicable)
Professional and respectful communication in all forums (including social media)
Constructive participation in organizational activities
Compliance with bylaws, policies, and governance decisions
Avoidance of defamatory, misleading, or divisive conduct
- Conflict of Interest
I agree to disclose any actual or potential conflicts of interest
I will not engage in activities that compromise the integrity of the organization
- Membership Review & Probation
I understand that my application/renewal is subject to review by the Membership Committee and approval by the Board of Directors
I acknowledge that my membership may be placed on probation (3–6 months) if concerns arise
I agree to comply with any conditions imposed during such probation
Memberships are for Calendar year – January 1 to December 31st with renewal grace period up to March 31st
- Disciplinary Actions
I understand that failure to meet the above standards may result in:
- Warning
- Probation
- Suspension
- Termination of membership
I acknowledge that due process will be followed, and I will have an opportunity to respond
- Confidentiality & Accuracy
I certify that all information provided is true and complete
I understand that false or misleading information may result in denial or termination of membership
I agree to maintain confidentiality of sensitive organizational matters
